Withdrawals
Withdrawals must be made in writing by emailing education@crockerart.org. If a student withdraws from a class a class credit, minus a $25 processing fee, is given upon requests in writing seven or more business days before the first class. A class credit of 50-percent of the class fee is given upon requests if received less than seven business days before the first meeting. Class credit can be used for future studio experiences. No credits are given on requests made or received on the day of the class or for classes with fees of less than $25. Make-up classes are not offered, and student replacement requests are not accepted within seven buisness days before the first class. Class registration fees cannot be prorated due to student absences. Full refunds are only given on the rare occasion that the Crocker must cancel a class.
Cancellations
The Crocker Art Museum reserves the right to cancel classes, to change instructors, and to make other changes that may become necessary to ensure a quality experience for participants. In the event that a class is canceled, we will contact all registrants and offer the option of a full class credit or refund.
Hold Harmless Agreement
In consideration of being permitted to participate in Crocker Art Museum programs, I agree to assume all risks connected therewith. I agree to release and discharge in advance the Crocker Art Museum, its officers, employees, and agents from any and all liability for personal injury. I also assume full responsibility for my behavior or the behavior of the minor child I have registered, and agree to pay for all damages to property or person caused by the aforementioned. If a participant’s behavior interferes with the program, I understand that expulsion from the program is a possible consequence.
Permission for Medical Treatment
In case of an accident or injury, I authorize a staff member of the Crocker Art Museum to call the 911 emergency number. I give my consent to any medical treatment felt necessary by an attending physician for the physical well-being of myself or for the minor child I have registered. I further understand that the responding medical emergency team will provide emergency treatment as they deem necessary for the safety and protection of myself or the minor child I have registered at my expense.
Consent to Photograph, Film, or Tape
I give the Crocker Art Museum permission to photograph, videotape, audio-record, and/or otherwise use my likeness, voice, and/or name in any and all uses by the Crocker Art Museum, including but not limited to educational, promotional, commercial, and advertising uses, including print, broadcast, and digital media, in perpetuity and for any purpose whatsoever. I understand that the images or media collected become the property of the Museum, and may also be included in the physical or electronic portfolios of the volunteers taking the photos. I give the Crocker Art Museum permission to use such material without any further claim on my part. I affirm that I am 18 years of age or older, and, if signing for a minor I am either a parent or legal guardian of the minor child with complete authority to grant this permission on the minor child’s behalf.
Age Agreement
Experiences are designed with the developmental stage of the participant in mind. By checking the box, I certify that I or my child is within the age range specified in the description. I understand that if I or my child are not within the age range Crocker will reverse my enrollment with a service fee of 50% of the cost of the class.
